Office Payment Policy

  • Examinations & office visits are payable at time of visit.
  • Glasses and contact lenses require a 50% deposit when ordered.
  • Glasses and contact lenses are payable in full when received.
  • We accept MasterCard and Visa.
  • Insurance Forms-We are participating providers for Blue Cross, BlueShield, Medicare and Medicaid, and other insurance's.

    • Our staff will gladly complete your insurance forms for additional insurance carriers to assist you in receiving reimbursement for our services.

    Pricing Information

    Our price list is long and contains pricing for nearly all types of contact lenses as well as eyeglass lenses. We have included an example below:

    Acuvue Daily wear disposable lenses, 2 week replacement:

  • $22.50 per pak.
  • $20.50 per pak when ordering 4 or more. Discount may not be combined with 3rd party plans.
  • Manufactures rebates also available up to $50 on initial order when ordering in bulk.
  • Free shipping when ordering 4 or more 6 Paks.

    • Criteria needed before ordering: For Perscription Glasses or Contact Lens, you will need:

  • A current Perscription (less then a 1 year old)
  • A credit Card Number
  • We can also check your files or call your Doctor to get your current Prescription
  • Your address, and phone number
